Steps to Follow for Debit Card Application

Download and print the application form via Adobe Acrobat.

If you are unable to do that, simply call our Customer Service Hotline at 1800 438 6088 and we will mail you an OCBC Debit Card application form.

You can also visit any OCBC Bank Branch to pick up an application form.

Please send your completed application form, with a copy of your NRIC (front and back)/Passport and a cheque of S$1,000 as the initial deposit (if applicable) to:
