• Have one copy of your NRIC, one copy of your child’s birth certificate or passport and two
   copies of your bank book (page with account number and name) or latest bank statement
   certified as true copies by the birth registration staff or a notary public;

• Complete Form BB1;

• Complete Form BB2.  Attach one certified true copy of your bank book/bank statement to Form
   BB2.  If you do not have the certified copy of your bank book/bank statement, go to the bank to
   endorse Part II of Form BB2;

• Complete OCBC CDA application form (applicable to a second, third or fourth child).  Attach
   certified true copies of your NRIC and your child's birth certificate to the OCBC CDA
   application form;

• Submit all the completed forms and certified copies to MCYS by post at:
